Home Health Nurse
Description

Provides, arranges for, teaches, or supervises nursing care of the sick and injured for patients in home health services. Provides health counseling and emotional support to individuals, families and groups. Consults with and refers families to appropriate personnel within the agency or other community services.

Responsibilities
  • Promote and maintain health of individuals, families and the community through teaching, counseling and appropriate preventive and rehabilitative measures.
  • Identify patients who have physical and emotional illnesses and /or disabilities through observation and analysis of records and help families secure appropriate medical, hospital or other needed care.
  • For patients / visits assigned, provide development of care plan, coordination of needed services and observation and monitoring of the patient’s response to care and treatment.
  • Provide skilled nursing care to patients requiring part time professional nursing
    service.
  • Carry out prescribed treatment to patients in their homes and other community settings.
  • Demonstrate, teach and supervise care to patients and families.
    Supervise L.P.N.’s and / or Home Health Aides. Develop and interpret care plans to such personnel.
  • Record observations, treatments given and other significant information. Maintain adequate records which reflect all aspects of patient care at all times.
  • Rotate weekend visit responsibility: one weekend in every quarter year.
  • Rotate one major and one minor holiday visit responsibility per year.
  • Consult about individual or family health problems with other professionals, such as physicians, dieticians, social workers and therapists.
  • Promote continuity of patient care by planning and exchanging information with physicians and other health and social agencies and updating the patient’s Nurse or Therapy Case Manager.
  • Help families to accept appropriate medical, hospital, nursing home, welfare or other care as necessary. Interpret extent and limitations of service available. Arrange referral and communicate pertinent family information to the agency.
  • Enhance professional growth and development by attending educational programs and in service meetings.
  • Carry out other duties as assigned.
Qualifications
  • Completion of a baccalaureate degree in nursing from a college or university accredited by the National League for Nursing is preferred OR Graduation from an accredited school of nursing.
  • Current license to practice as a Registered Nurse in the State of Illinois.
  • Current Illinois Drivers License
  • Basic computer skills
